Key Responsibilities

• Provide direct, person-centered care and support to adults with disabilities in line with their care plans.
• Assist individuals with daily living activities, including personal care, mobility, and medication management.
• Develop and maintain positive relationships with clients, their families, and other healthcare professionals.
• Support individuals in engaging in community activities, social outings, and pursuing personal interests.
• Accurately maintain client records, progress notes, and incident reports.
• Promote independence, choice, and dignity for all individuals receiving care.

Requirements

• Previous experience in a social care or support role, preferably with adults with disabilities.
• Excellent communication and interpersonal skills, with the ability to build rapport.
• A genuine passion for supporting vulnerable individuals and promoting their well-being.
• Ability to work effectively as part of a team and independently.
• Willingness to undergo a DBS check and provide satisfactory references.
• Basic IT literacy for record-keeping purposes.
