This is an excellent opportunity to join the Transition to Adult Social Care Service, supporting young people as they move from Children's Services into Adult Social Care. • Starting Salary: £37,280 per year, increasing annually up to £41,777
• Hours: Full-time (37 hours per week)

Role Purpose

As a Social Worker within the Transition to Adult Social Care Team, you will support young people and their families throughout the transition from Children's Services into Adult Social Care, ensuring every transition is well-planned, person-centred, and focused on achieving independence.

You will carry out Care Act Assessments, reviews, Mental Capacity Assessments, safeguarding enquiries, and Court of Protection work while supporting individuals to live fulfilling and independent adult lives.

Key Responsibilities

• Carry out strengths-based Care Act Assessments, reviews, support planning, and risk assessments.
• Complete Mental Capacity Assessments, Safeguarding Enquiries, and Court of Protection work.
• Manage a varied caseload, ensuring support plans remain personalised, outcome-focused, proportionate, and regularly reviewed.
• Work directly with young people and families to encourage independence and informed decision-making.
• Complete Carers Assessments in accordance with statutory responsibilities.
• Respond to referrals and urgent situations through the team's Triage system, including crisis intervention where required.
• Prepare clear, accurate, and professional assessments and reports to support decision-making.
• Work collaboratively with partner agencies to deliver coordinated, person-centred support.
• Contribute to care planning, service decision-making, and resource allocation.
• Maintain accurate, timely, and compliant case records in accordance with legislation and organisational policy.

What We're Looking For

We are seeking Social Workers who are committed to person-centred, strengths-based practice and who are passionate about supporting young people as they transition into Adult Social Care.

• Recognised Social Work qualification.
• Current registration with Social Work England.
• Experience, or a strong interest, in Adult Social Care.
• Ability to assess and manage risk while making sound professional decisions.
• Excellent communication skills, including experience supporting individuals with varied communication needs.
• Ability to work collaboratively across multiple agencies while maintaining a strong social work perspective.
• Commitment to promoting independence, dignity, inclusion, and positive outcomes.

You will work alongside experienced Social Workers and Adult Social Care Practitioners within the wider Learning Disabilities Service.

• Education providers
• Health professionals
• Community and local support services

Additional Information

Please note that Skilled Worker visa sponsorship may be considered for suitably qualified overseas Social Workers who:

• Are already living in the UK.
• Hold a recognised Social Work qualification.
• Are registered with Social Work England.

Sponsorship will be assessed during the recruitment process. Unfortunately, relocation or resettlement support cannot be provided.

Due to the rural nature of Somerset and limited public transport, applicants must have access to a vehicle for work purposes.

Person Specification

Criteria

Knowledge and understanding of Social Care practice in line with Social Work England standards and relevant legislation

Ability to speak fluent English (Immigration Act 2016)

Understanding of community and non-statutory social care resources

Awareness of government guidance and legislative developments affecting social care

Relevant multidisciplinary social care experience

Experience working collaboratively with Social Care, Health, Education, or Voluntary agencies

Experience supporting individuals with Learning Disabilities

Supervisory experience

Experience working alongside health professionals

Degree in Social Work or equivalent recognised qualification

Registration with Social Work England

Demonstrates empathy, self-awareness, sensitivity, and respect for others

Ability to prioritise workload and meet competing deadlines

Resilient, adaptable, and able to manage challenging situations positively

Willingness to embrace organisational change
