Adult Social Worker – Community Care Team
Liverpool, Merseyside | £32.82 per hour (Umbrella)
We are looking for an experienced Adult Social Worker to join a Community Care Team in Liverpool, supporting vulnerable adults to live safely, independently and with dignity.
Key Responsibilities
* Complete Care Act 2014 assessments, reviews and support plans.
* Undertake mental capacity assessments and contribute to best-interest decisions.
* Respond to safeguarding concerns and undertake Section 42 enquiries.
* Develop strengths-based, personalised care and support plans.
* Assess and manage risks relating to abuse, neglect and vulnerability.
* Work closely with NHS services, Occupational Therapists, mental health teams, housing and voluntary organisations.
* Support adults, families and carers with information, advice and access to appropriate services.
* Maintain accurate case records and prepare reports for panels and legal proceedings.
* Promote independence, choice, wellbeing and positive outcomes.
Essential Requirements
* Recognised Social Work qualification (BA/BSc/MA or equivalent).
* Current Social Work England registration.
* Strong knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.
* Experience in adult social care, assessments, care planning and safeguarding.
* Enhanced DBS clearance.
* Full UK driving licence and access to a vehicle for community visits, where required.
