About the Role

Key Responsibilities
- Conduct comprehensive assessments of individual and family needs, identifying risks and safeguarding concerns.
- Develop, implement, and review person-centred care and support plans in collaboration with clients and stakeholders.
- Provide emotional support, practical advice, and signposting to relevant community services and resources.
- Liaise effectively with other agencies, including healthcare providers, educational institutions, and legal services, to ensure coordinated care.
- Maintain accurate and confidential case records, documenting all interventions, progress, and outcomes in line with professional standards and legal requirements.
- Participate in team meetings, case conferences, and professional development activities to enhance practice and contribute to service improvement.
Requirements
- Recognised Social Work qualification (e.g., BA in Social Work, DipSW, CQSW) and current registration with the relevant professional body.
- Proven experience working within a statutory or community social care setting, with a strong understanding of relevant legislation and policies.
- Demonstrable experience in safeguarding vulnerable adults and children, with a commitment to promoting their welfare.
- Excellent communication, interpersonal, and negotiation skills, with the ability to build rapport and trust with diverse client groups.
- Proficiency in case management systems and standard office software, with strong report-writing abilities.
