About the Role

Our client is seeking a compassionate and proactive Support Care Navigator to join their fully remote team, dedicated to serving the Southampton community. You will leverage digital communication tools to assess needs, provide information, and connect clients with the right resources, ensuring they receive the care they deserve. Key Responsibilities

• Conduct remote needs assessments to understand client requirements for social and health support.
• Provide information and guidance on available services, benefits, and community resources.
• Facilitate referrals and connections between clients and appropriate service providers through digital means.
• Monitor client progress and follow up to ensure satisfaction and the effectiveness of support provided.
• Maintain accurate and confidential client records within the digital case management system.
• Offer emotional support and encouragement to clients navigating challenging circumstances.

Requirements

• Previous experience in a care, support, or advisory role.
• Excellent remote communication and active listening skills.
• Proficiency with digital communication tools (video conferencing, phone, email) and CRM systems.
• Strong organizational and time-management abilities for managing a remote caseload.
• Empathy, patience, and a genuine desire to help others.
• Familiarity with local services and resources within the Southampton area is advantageous.
