About the Role

Key Responsibilities

• Coordinate and oversee the delivery of personalized care plans for a caseload of clients.

Requirements

• Experience in a social care , healthcare, or support role is essential.
• Strong understanding of care principles, safeguarding, and person-centered support.
• Excellent communication, organizational, and time-management skills.
• Proficiency in using digital tools for communication, record-keeping, and remote monitoring.
• Ability to work independently and manage a remote caseload effectively.
