About the Role
Our client is seeking a proactive and organised Support Coordinator to join their disability services team in Derby , operating on a hybrid model that ensures flexibility while maintaining essential community engagement. This role is vital in empowering individuals with disabilities to access the services and support they need to achieve their personal goals and live fulfilling lives. You will work closely with clients, their families, and a network of service providers to develop, implement, and review personalised support plans. This position requires excellent communication, problem-solving skills, and a deep commitment to person-centred care, contributing to a service that champions independence and inclusion within the local Derby community.
Key Responsibilities
- Conduct comprehensive needs assessments for individuals with disabilities.
- Develop, implement, and regularly review personalised support plans in collaboration with clients and stakeholders.
- Coordinate the delivery of a range of supports, including allied health services, personal care, and community access.
- Liaise with external service providers, government agencies, and community organisations to secure appropriate resources.
- Monitor client progress and satisfaction, making necessary adjustments to support plans.
- Maintain accurate and confidential client records and case notes.
Requirements
- Experience in disability support, case management, or a related field.
- Strong understanding of the NDIS framework and disability service provision.
- Excellent communication, interpersonal, and negotiation skills.
- Proficiency in IT systems for case management and reporting.
- Ability to work independently and collaboratively within a hybrid work structure.
