Support Coordinator

About the Role

Our client is seeking a motivated Support Coordinator to join their team in Derby , operating on a hybrid model. This role is central to ensuring that individuals with complex needs receive appropriate and timely support services. You will be responsible for assessing needs, developing personalised support plans, and coordinating with a network of care providers, agencies, and family members to deliver seamless care. This position requires strong organisational skills, excellent communication, and a deep understanding of the social care landscape to effectively advocate for service users and facilitate their independence.

Key Responsibilities

• Conduct comprehensive needs assessments for individuals requiring support services.
• Develop, implement, and review personalized support plans in collaboration with service users and their families.
• Coordinate and manage the delivery of support services from various providers, ensuring quality and efficiency.
• Act as a key point of contact for service users, their families, and external agencies.
• Monitor the effectiveness of support plans and make necessary adjustments to meet evolving needs.
• Maintain accurate records and reports related to client assessments, plans, and service delivery in Derby .

Requirements

• Proven experience in a support coordination , case management, or social work role.
• In-depth knowledge of relevant legislation, policies, and best practices in social care.
• Excellent communication, negotiation, and interpersonal skills.
• Strong organisational and time-management abilities, with the capacity to manage multiple cases.
• Proficiency in using IT systems for record-keeping and communication.
• A relevant qualification in Health and Social Care, Social Work, or a related field is highly desirable.
