Key Responsibilities
- Develop, implement, and monitor individual support plans tailored to client needs and preferences.
- Coordinate with healthcare professionals, support agencies, and families to ensure integrated care delivery.
- Manage caseloads effectively, ensuring all clients receive timely and appropriate support.
- Conduct regular reviews of support plans, making necessary adjustments based on client progress and feedback.
- Maintain accurate and confidential records of client information, progress, and outcomes.
- Provide guidance and support to care staff, ensuring they understand and adhere to support plans and policies.
Requirements
- Experience in a social care, health, or support coordination role.
- Understanding of needs assessment processes and care planning principles.
- Excellent organisational and time-management skills.
- Strong interpersonal and communication abilities, with the capacity to build rapport with diverse individuals.
- Knowledge of safeguarding vulnerable adults and children policies and procedures.
- Proficiency in using IT systems for record-keeping and communication.
