About the Role
Our client is seeking an empathetic and organised Support Coordinator to join their vital services team in Cardiff . This role is focused on empowering individuals within the community by facilitating access to essential services and support networks. You will be responsible for managing caseloads, developing personalised care strategies, and working collaboratively with a network of external agencies. This is a field-based position requiring significant travel within the Cardiff area to meet clients and provide direct support. The successful candidate will be passionate about community welfare and dedicated to improving the lives of those they assist.
Key Responsibilities
- Conduct thorough needs assessments for clients to identify support requirements and goals.
- Develop, implement, and monitor personalised support plans in collaboration with clients and stakeholders.
- Coordinate and arrange access to a range of services, including healthcare, housing, and social activities.
- Maintain detailed and accurate records of client interactions, progress, and outcomes.
- Build and maintain strong working relationships with partner agencies and community organisations.
- Provide direct support and advocacy to clients, ensuring their voices are heard and needs are met.
Requirements
- Experience in a similar support coordination or social care role.
- Strong understanding of the social care sector and available community resources in Wales .
- Excellent organisational and time-management skills, with the ability to manage a varied workload.
- Proficiency in IT systems, including case management software and standard office applications.
- Full UK driving licence and access to a vehicle for travel.
- Exceptional communication and interpersonal skills, with a compassionate approach.
