About the Role

Our client is looking for a proactive and organized Support Coordinator to join their dynamic team in Manchester . In this hands-on role, you will play a crucial part in ensuring the smooth delivery of support services to individuals with diverse needs, including those with disabilities, mental health challenges, and older adults. You will be responsible for managing a caseload, coordinating support workers, and liaising with families and external professionals to facilitate effective care plans. This is a field-based position requiring regular client visits and community engagement.

Key Responsibilities

• Manage and coordinate the day-to-day delivery of support services to a caseload of clients.
• Schedule and allocate tasks for support workers, ensuring appropriate matches and coverage.
• Conduct initial client assessments and develop personalized support plans in line with their needs and preferences.
• Build and maintain strong working relationships with clients, their families, and a network of external agencies.
• Monitor the quality of service delivery, respond to client feedback, and address any issues or concerns promptly.
• Maintain accurate client records and support worker logs in the company's database.

Requirements

• Previous experience in a support coordination or care management role is essential.
• Strong understanding of the challenges faced by individuals within the community care sector.
• Excellent organizational and time-management skills, with the ability to manage multiple priorities.
• Proficiency in communication and interpersonal skills to effectively engage with clients and stakeholders.

• A full UK driving license and access to a vehicle for client visits is required.
