Key Responsibilities
- Develop, monitor, and review individual support plans for clients, ensuring they meet evolving needs.

- Act as a primary point of contact for clients, their families, and external agencies, providing information and resolving queries.
- Maintain comprehensive and up-to-date client records and administrative documentation.
- Facilitate effective communication channels between all parties involved in client care.
- Identify service gaps and propose solutions to enhance the overall support system.
Requirements
- Proven experience in a coordination or management role within a social care or similar setting.
- Strong organisational, planning, and time-management skills.
- Excellent communication, negotiation, and problem-solving abilities.
- Proficiency in using case management software and digital collaboration tools.
- Ability to work independently and manage a varied caseload in a remote environment.
