About the Role
Our client is seeking a proactive and organised Support Coordinator to join their dynamic team in Bradford . This role is instrumental in ensuring seamless delivery of support services to individuals within the community, involving coordination of care plans, staff rotas, and client communications. The position offers a hybrid working model, blending office-based duties with the flexibility of remote work. You will be a key point of contact, ensuring that our client’s services meet the highest standards of quality and care, while contributing to the efficient operation of their West Yorkshire branch.
Key Responsibilities
- Coordinate the scheduling and deployment of support workers to meet client needs effectively.
- Act as a primary point of contact for clients, families, and support staff, addressing queries and concerns.
- Maintain accurate client records, support logs, and staff information systems.
- Assist in the recruitment, onboarding, and ongoing support of the care team.
- Monitor service quality and implement improvements based on feedback and performance data.
- Liaise with external agencies and healthcare professionals to facilitate integrated care.
Requirements
- Experience in a coordination, scheduling, or administrative role, preferably within the social care sector.
- Excellent organisational and time-management skills with meticulous attention to detail.
- Strong IT proficiency, including experience with scheduling software and Microsoft Office Suite.
- Effective communication and interpersonal abilities to build rapport with diverse stakeholders.
- Ability to work autonomously and collaboratively in a hybrid environment.
