Support Worker (Complex Needs)

About the Role

Our client seeks a highly motivated and skilled Support Worker experienced in handling complex needs to join their team serving the Bristol area. This role involves providing dedicated, person-centred care and support to individuals with challenging behaviours, mental health issues, learning disabilities, or physical disabilities, enabling them to live as independently and safely as possible. You will be instrumental in implementing tailored support plans, managing challenging situations with professionalism and empathy, and working collaboratively with a multidisciplinary team. The hybrid nature of this role allows for effective community-based support combined with administrative tasks and team coordination, ensuring comprehensive care delivery.

Key Responsibilities

• Provide direct support to individuals with complex needs, assisting with personal care, daily living tasks, and community integration.
• Develop and implement individualised support plans, adapting strategies to meet evolving client needs and reduce risk.
• Manage and de-escalate challenging behaviours using positive and person-centred approaches.
• Administer medication and support clients with healthcare routines as per care plans.
• Liaise effectively with families, carers, healthcare professionals, and external agencies to ensure coordinated care.
• Maintain accurate and detailed records of client progress, incidents, and interventions.

Requirements

• Significant experience supporting individuals with complex needs , including challenging behaviours and multiple support requirements.
• Strong understanding of safeguarding vulnerable adults and children, and relevant legislative frameworks.
• Excellent communication, observational, and problem-solving skills.
• Ability to work calmly and effectively under pressure and respond appropriately to emergencies.
• Proficiency in using digital tools for record-keeping and communication.
• Relevant qualifications (e.g., NVQ/QCF Level 3 in Health and Social Care) are highly desirable.
