Key Responsibilities

• Conduct assessments of needs and risks for vulnerable adults.
• Develop and implement individualised support and care plans.
• Provide direct support and guidance to clients in their homes and community.
• Liaise effectively with families, carers, and other professionals involved in the client's care.
• Ensure the safety and well-being of clients, reporting any concerns promptly.
• Maintain accurate and confidential records of client interactions and progress.

Requirements

• Experience in social care, support work, or a related field.
• Knowledge of safeguarding vulnerable adults and relevant legislation.
• Strong communication, interpersonal, and problem-solving skills.
• Ability to work with empathy and professionalism.
• Willingness to undertake further training and development.
• A valid driving license and access to a vehicle may be necessary for client visits in and around Norwich .
