Job Description
Responsibilities
- Co-ordinating Youth Inclusion Panel referrals, assessments and meetings, ensuring effective multi-agency decision-making and support planning.
- Building and maintaining positive relationships with children, families, schools and partner agencies to achieve the best possible outcomes.
- Preparing and monitoring case records, reports and management information, ensuring accuracy and compliance with relevant policies and procedures.
- Representing the service at meetings and reviews, providing professional advice and ensuring actions are followed through.
- Supporting continuous service improvement through data analysis, reporting and engagement in training and partnership activities.
Requirements
- A qualification in Social Care, Health, Youth Work or a related field.
- Experience working with children, young people and families, ideally within a multi-agency or safeguarding environment.
- Strong communication and relationship-building skills, with the ability to engage a wide range of stakeholders.
- Excellent organisational skills and the ability to manage competing priorities in a fast-paced environment.
- Strong report-writing, problem-solving and decision-making skills, with confidence using databases and Microsoft Office applications.
