About the Role
Our client is looking for a compassionate and organised Care Coordinator to oversee services for elderly residents in Edinburgh . This crucial role ensures that older adults receive high-quality, personalised care that meets their individual needs and promotes their independence and well-being. You will work closely with clients, their families, healthcare professionals, and care staff to develop, implement, and monitor care plans, ensuring a seamless and supportive care experience within the community setting.
Key Responsibilities
- Conduct comprehensive needs assessments for elderly clients, developing tailored care and support plans.
- Coordinate the provision of care services, including arranging visits from support workers, healthcare professionals, and therapists.
- Regularly review and update care plans in consultation with clients, families, and the care team to ensure ongoing effectiveness.
- Liaise with external agencies, such as GPs, hospitals, and social services, to ensure integrated care delivery.
- Manage and support a team of care workers, providing guidance and ensuring high standards of service delivery.
- Maintain accurate and confidential client records and ensure compliance with regulatory standards and policies.
Requirements
- Proven experience in a care setting, preferably within elderly care or community services.
- Strong understanding of the needs of older adults and best practices in care provision.
- Excellent organisational, communication, and interpersonal skills.
- Ability to manage multiple cases, prioritise effectively, and work collaboratively with diverse stakeholders.
- Relevant qualification in health and social care (e.g., NVQ Level 3 or above) is desirable.
