About the Role
Our client is seeking a proactive and empathetic Care Coordinator to manage and oversee support services for elderly individuals in the Edinburgh area. This hybrid role involves coordinating care packages, liaising with families and healthcare professionals, and ensuring the highest standards of care are met, with some elements of remote administration and planning alongside client interaction. You will play a crucial role in assessing needs, developing personalized care plans, and ensuring continuity of care, promoting independence and quality of life for older adults. This position offers the chance to lead within a dedicated community care team.
Key Responsibilities
- Conduct comprehensive assessments of care needs for elderly clients, developing tailored care plans.
- Coordinate the delivery of home care services, including scheduling staff and managing rotas.
- Act as the primary point of contact for clients, their families, and healthcare professionals, ensuring effective communication.
- Monitor the quality of care provided, undertaking regular reviews and making adjustments to care plans as needed.
- Ensure compliance with all relevant regulations, policies, and quality standards.
- Manage and support a team of care staff, providing guidance and supervision.
Requirements
- Proven experience in care management, coordination, or a similar role within health or social care.
- Strong understanding of the needs of the elderly and principles of person-centered care.
- Excellent organizational, planning, and time-management skills.
- Effective communication and interpersonal abilities, with the capacity to build rapport with diverse individuals.
- Proficiency in using care management software and standard office applications.
- Relevant qualification (e.g., Diploma in Health and Social Care Level 5) is desirable.
