Care Coordinator - Elderly Support

About the Role

Our client is looking for an organized and compassionate Care Coordinator to manage and deliver high-quality support services for elderly individuals in Milton Keynes . You will be responsible for assessing needs, developing care plans, and coordinating services to ensure seniors can live safely and comfortably in their own homes. This role is pivotal in enhancing the quality of life for older adults, working within a dynamic Community & Social Care team that values dedication and professional excellence. You will be a key contact point for clients, their families, and a network of care providers in the Buckinghamshire area.

Key Responsibilities

• Conduct thorough assessments of elderly clients' needs, including physical, emotional, and social requirements.
• Develop, implement, and regularly review personalized care and support plans.
• Coordinate the provision of services, including home care, social activities, and medical appointments.
• Liaise with families, healthcare professionals, and external agencies to ensure seamless care delivery.
• Monitor client well-being and satisfaction, responding proactively to any changes or concerns.
• Manage a team of support workers, providing guidance and ensuring high standards of care.

Requirements

• Proven experience in a care coordination or similar role within Community & Social Care .
• Strong understanding of the needs and challenges associated with elderly care.
• Excellent organizational, planning, and time-management skills.
• Effective communication and interpersonal abilities, with empathy and patience.
• Knowledge of relevant care regulations and best practices.
• Ability to work both independently and as part of a collaborative team in Milton Keynes .
