About the Role
Our client is looking for a compassionate and organized Care Coordinator to manage and deliver high-quality support services for elderly individuals in and around Leeds . This role involves assessing client needs, developing personalized care plans, and coordinating a team of care professionals to ensure clients receive the best possible support in their own homes or in residential settings. You will play a key role in enabling older adults to maintain their independence and quality of life, working within a framework that values dignity, respect, and individualised care. This hybrid role combines essential community-based work with administrative duties, offering a fulfilling career in a growing sector.
Key Responsibilities
- Conduct comprehensive needs assessments for elderly clients, developing tailored care and support plans.
- Coordinate the scheduling and deployment of care staff, ensuring appropriate matching of skills to client needs.
- Monitor the effectiveness of care plans and make necessary adjustments in consultation with clients and families.
- Liaise with healthcare professionals, social services, and other stakeholders to ensure seamless service delivery.
- Maintain accurate client records and ensure compliance with regulatory standards and quality assurance frameworks.
Requirements
- Proven experience in a care coordination, case management, or similar role within the health or social care sector, particularly with the elderly.
- Strong understanding of care planning, risk assessment, and safeguarding principles.
- Excellent organizational, communication, and interpersonal skills.
- Ability to manage a caseload efficiently and work effectively both independently and as part of a team.
- A relevant qualification in health and social care (e.g., NVQ Level 3 or 4) is highly desirable.
