💙 About the Role

As a Care Coordinator, you’ll be responsible for coordinating care packages, managing staff rotas, supporting care workers, and ensuring service users receive the right care at the right time.

🔑 Key Responsibilities

• Coordinate and manage daily care schedules and staff rotas.

• Allocate care workers to service users, ensuring continuity of care wherever possible.

• Respond to changes in service users’ needs and staffing requirements.

• Maintain accurate care records, schedules and documentation.

• Liaise with service users, families, care staff and healthcare professionals.

• Monitor attendance, punctuality and service delivery.

• Support recruitment, induction and ongoing communication with care staff.

• Ensure care services meet safeguarding requirements and relevant quality standards.

• Previous experience in care coordination or a similar role within the care sector.

• Excellent organisational and time-management skills.

• Strong communication and problem-solving abilities.

• The ability to work well under pressure in a fast-paced environment.

• A good understanding of care planning, safeguarding and person-centred care.
