About the Role

Our client is seeking a compassionate and resourceful Care Navigator to join their team, focusing on supporting individuals in and around Belfast . This role is primarily remote, allowing you to connect with clients, families, and service providers through digital channels, ensuring seamless access to care and support services. You will be instrumental in guiding individuals through complex care systems, helping them to understand their options and access the resources they need to enhance their quality of life. As a vital link in the support network, you will work to empower individuals and foster independence, all from a remote-first operational base. Key Responsibilities

• Provide information, advice, and guidance on available care and support services.
• Assist clients in navigating local health and social care systems.
• Conduct remote needs assessments and develop personalized action plans.

• Monitor client progress and follow up to ensure satisfaction and effectiveness of services.
• Maintain accurate and confidential client records using digital platforms.

Requirements

• Experience in a similar role within health, social care, or a related field.
• Excellent communication, listening, and problem-solving skills.
• Proficiency in using digital communication tools, CRM systems, and virtual collaboration platforms.
• Strong understanding of the local service landscape in Belfast and Northern Ireland .
• Ability to work independently, manage time effectively, and maintain a high level of professionalism remotely.
