The Help Hub brings together a wide range of Council services, over 20 voluntary, community and social enterprise (VCSE) organisations, and key statutory partners including Adult Social Care, the NHS, Children's Services and the Police. Together, we work to improve the social, physical, emotional and financial wellbeing of local residents, helping people access the support they need to thrive.
About the Role
Did you know that the greatest factors affecting people's health are socio-economic, behavioural and environmental? Helping people overcome the challenges in their lives leads to better health outcomes.
As a Community Connector / Social Prescriber, you will work predominantly in GP surgeries and some community settings across the districts. Working alongside clinical colleagues in primary care, your role will be to support those who present with a social need or whose medical condition is being made worse by social factors.
What you'll be doing…
Taking referrals from colleagues in primary care and from the Council's Help Hub. Interviewing residents and assessing their needs. Supporting residents in the medium term to access the help and support they need. Liaising with a variety of statutory and voluntary agencies to create an effective plan of support for residents in need. Joining a team of 23 established social prescribers who are happy to share their knowledge and support one another.About You
You will be a compassionate and resilient individual with a genuine passion for supporting people to improve their wellbeing. With experience of delivering support in a community setting, including working with people in distress, you will ideally be acquainted with the benefits system and have a rudimentary knowledge of welfare rights.
You will be an effective communicator with the ability to establish and maintain positive working relationships with peers, colleagues and other professionals. Personal resilience and a sense of humour are essential to this role, as is the ability to travel independently across the locality on a regular basis.
