Community Inclusion Officer

About the Role

Our client is seeking a dynamic and community-focused Community Inclusion Officer to join their team in Derby . This role is dedicated to promoting independence, social participation, and well-being for individuals within the community, particularly those who may face barriers to inclusion. You will work directly with clients, identifying their aspirations and connecting them with local resources, activities, and opportunities. This position requires a proactive approach, excellent networking skills, and a deep understanding of community assets. Key Responsibilities

• Conduct needs assessments and develop personalised plans to support community inclusion goals.
• Actively identify and develop links with local community groups, services, and opportunities.
• Facilitate client participation in social, recreational, educational, and vocational activities.
• Provide ongoing support and encouragement to clients as they engage with community resources.
• Advocate for clients’ needs and help overcome barriers to their full inclusion.
• Maintain accurate records of client progress, activities, and outcomes achieved.

Requirements

• Experience in social care, community development, or a related field is essential.
