An opportunity has arisen for a Community Mental Health Practitioner looking to join a great team, providing specialist mental health care to the population of Bury. The Secondary Care CMHT support residents with severe and enduring mental health conditions.
This an exciting time for Bury SMHT. We are currently undergoing a transformation within our service as part of the National Transformations of community services and the local Living Well developments. It is an opportunity to be part of our service development, with planned investment in our staff and new role development.
The CMHT work with people aged 18 and over with severe and enduring mental health needs.
Mental Health Practitioners are responsible for providing holistic assessment, interventions, risk assessments and care plans for people on their caseload. As a nurse in the team. You will be responsible for administering depot medication for people who are unable to attend the depot clinic.
Main duties of the job
To act as the community Mental Health Practitioner and key worker responsible for the care of an identified group of clients, being part of a service that is committed to recovery and social inclusion.
To provide supervision to an identified group of staff and students on placement with the team.

Person specification
Education/Qualifications
- Professional qualification Social Work / Mental Health Nursing / Occupational Therapy
- Registration with HCPC / NMC
- Experience in community mental health work
- Willingness to undertake further training relevant to the practitioner role resulting from legislative or service changes, as appropriate.
- Evidence of post-basic qualification and training
- PSI Training
- MSLAP or equivalent Mentor training
- AMHP
Experience
- Knowledge of CPA policy
- Experience of supervision
- Experience of leading a team / ’acting up’.
Knowledge
- Experience in working with people with moderate to severe mental health needs
- Extensive knowledge / experience in social inclusion and recovery model of care.
- Knowledge of mental health legislation
- Experience of MDT working
- Experience of managing or participating in change
Skills and Abilities
- Ability to undertake the key worker role
- Evidence of ability to provide specialist mental health assessment and risk assessment / management and be accountable for decision making
- Excellent written and interpersonal communication skills
- Ability to manage own emotions and deal with others in highly stressful situations
- Ability to engage with and provide therapeutic interventions for this client group and carers
- Ability to prepare, record and complete relevant care records and reports to a professional standard
- Evidence of ability to manage a diverse and complex caseload, prioritise own time and work under pressure
- Ability to coordinate and delegate the work of junior staff
- Ability to value diversity and respond constructively to discriminatory behaviour

Work Related Citcumstances
- Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
- Willing to carry out all duties and responsibilities of the post in accordance with the Trust’s Equal Opportunities and Equality and Diversity policies
