Key Responsibilities

• Undertake outreach activities to identify and engage with individuals and families requiring support within the Belfast community.
• Conduct needs assessments to understand client circumstances and develop personalised support plans.
• Provide information, advice, and guidance on available services, benefits, and entitlements.
• Facilitate referrals to internal and external agencies, including housing, health, and employment services.
• Develop and maintain positive relationships with community stakeholders, local authorities, and partner organisations.
• Organise and deliver workshops or information sessions to raise awareness of available support.

Requirements

• Experience working in a social care, community development, or outreach role.
• Excellent communication, negotiation, and interpersonal skills.
• Ability to work independently and manage your own schedule effectively, balancing field visits with office-based or remote tasks.
• Good understanding of the social challenges and support services available in Northern Ireland .
• Proficiency in using IT systems for case management, data recording, and communication.
• A full driving licence and access to a vehicle for community visits is essential.
