About the Role
Our client is seeking a dedicated Community Outreach Worker to engage with vulnerable individuals and families in the Leeds area. This role involves building trust, assessing needs, and connecting people with essential social care services and community resources. You will be instrumental in fostering positive relationships within the community, providing support, guidance, and advocacy. This position offers a dynamic blend of fieldwork and office-based coordination, contributing directly to the well-being of the local population and requiring a strong understanding of the community and social care landscape .
Key Responsibilities
- Conduct outreach activities in various community settings to identify and engage with individuals in need of support.
- Assess client needs holistically, providing information, advice, and guidance on available services.
- Develop and maintain relationships with local community groups, charities, and statutory services to enhance referral pathways.
- Support clients in accessing services, accompanying them to appointments or meetings where necessary.
- Maintain accurate client records and produce reports on outreach activities and outcomes.
- Advocate for clients' needs, ensuring their voices are heard and their rights are protected.
Requirements
- Experience in a community-based role, social work, or a related field.
- Strong understanding of the challenges faced by vulnerable individuals and communities.
- Excellent interpersonal and communication skills, with the ability to engage diverse populations.
- Proficiency in assessment techniques and basic counseling skills.
- Ability to work autonomously and manage a flexible schedule, including some evening and weekend work.
- A valid driving license and access to a vehicle for travel within the Leeds region is highly desirable for fieldwork aspects.
