About the Role
Our client is seeking an enthusiastic and dedicated Community Outreach Worker to join their team in Bradford . You will be responsible for engaging with individuals and families within the community, assessing their needs, and connecting them with appropriate social care services and resources. This role requires a proactive approach to building relationships and fostering trust with diverse groups. You will play a key part in improving access to support and enhancing the overall well-being of residents in the West Yorkshire area, working within a flexible hybrid model.
Key Responsibilities
- Develop and maintain positive relationships with individuals, community groups, and local organisations.
- Conduct outreach activities to identify individuals and families in need of social care support.
- Assess client needs and provide information, advice, and guidance on available services and resources.
- Facilitate referrals to appropriate social care, health, and community support services.
- Organise and deliver community-based information sessions and events to promote awareness of services.
- Maintain accurate records of outreach activities, client interactions, and referrals made.
Requirements
- Experience in community work, social care, or a related field.
- Excellent communication, listening, and interpersonal skills.
- Ability to engage effectively with diverse populations and build rapport.
- Strong organisational skills and the ability to manage outreach activities.
- Knowledge of local services and resources in the Bradford area is advantageous.
- Willingness to travel within the community and occasionally work flexible hours.
