Community Support Officer - Vulnerable Adults

About the Role

Our client is seeking a dedicated Community Support Officer to join their vital team in Birmingham , providing essential services to vulnerable adults. This hybrid role offers a blend of field-based support and remote administrative duties, focusing on empowering individuals to maintain their independence within their own homes and communities. You will work closely with service users to assess their needs, develop personalised care plans, and connect them with relevant resources and support networks. Key Responsibilities

• Conduct comprehensive needs assessments for vulnerable adults, identifying support requirements and personal goals.
• Develop, implement, and regularly review person-centred support plans in collaboration with service users and their families.
• Coordinate and facilitate access to a range of community services, including healthcare, social activities, and educational opportunities.
• Provide direct support and guidance to individuals, promoting independence, self-advocacy, and social inclusion.
• Maintain accurate and up-to-date electronic records, ensuring confidentiality and adherence to data protection regulations.
• Build and maintain strong working relationships with statutory and voluntary organisations within the West Midlands region to enhance service provision.

Requirements

• Experience in a social care, community support, or similar role, with a focus on working with vulnerable adults.
• Understanding of safeguarding principles and procedures for adults at risk.
• Excellent communication, interpersonal, and negotiation skills, with the ability to engage effectively with diverse individuals.
• Proficiency in using IT systems for case management, record-keeping, and remote communication.
• A valid UK driving license and access to a vehicle for travel within the Birmingham area is preferred for fieldwork.
• Relevant qualifications such as a Diploma in Health and Social Care (Level 3 or above) are advantageous.
