About the Role

You will be instrumental in developing and maintaining strong relationships with clients, local authorities, and partner organisations across Cardiff and the surrounding areas. Key Responsibilities

• Conduct assessments of client needs and develop tailored support plans to address their social, emotional, and practical challenges.
• Facilitate access to essential services, including housing, employment, education, and healthcare, acting as a key advocate for clients.
• Provide emotional support and guidance, empowering clients to build resilience and self-sufficiency.
• Collaborate effectively with a range of stakeholders, including statutory services, voluntary organisations, and community groups.
• Maintain accurate and confidential case notes and progress reports, utilising digital systems for efficient record-keeping.

Requirements

• Experience in social work, community development, or a related field is preferred.
• Strong understanding of social welfare issues and available support services within Wales .
• Excellent communication, negotiation, and problem-solving skills.
• Ability to work autonomously and manage a caseload effectively, with strong organizational skills.
• A driving license and access to a vehicle for community visits is essential for this hybrid role.
