Key Responsibilities

• Conduct comprehensive needs assessments for individuals seeking support within the community.
• Develop and implement tailored support plans, linking service users with appropriate resources and services.
• Liaise with external agencies, including housing providers, health services, and employment support organizations.
• Provide practical assistance and guidance to service users in accessing benefits and entitlements.
• Monitor progress and review support plans regularly, making adjustments as needed to ensure effectiveness.
• Maintain accurate and confidential case records and prepare reports for management and stakeholders.

Requirements

• Proven experience in a social work, community development, or similar support role.
• Strong understanding of welfare rights, housing legislation, and local support services.
• Excellent assessment, planning, and interpersonal skills.
• Ability to manage a caseload effectively and work collaboratively with diverse stakeholders.
• Proficiency in using case management software and standard office applications.
• A valid UK driving license and access to a vehicle for community outreach is beneficial.
