Key Responsibilities
- Conduct needs assessments for individuals requiring community support, identifying specific goals and challenges.
- Develop and implement individualised support plans, working collaboratively with service users and their families.
- Provide practical assistance and emotional support to service users in their own homes and communities.
- Facilitate access to relevant community resources, such as employment services, health clinics, and social groups.
- Monitor the progress of service users and review support plans regularly to ensure effectiveness.
- Maintain accurate and confidential records of all interactions and interventions.
Requirements
- Experience in a care or support role, preferably within community settings.
- Understanding of the challenges faced by individuals requiring support within the community.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust.
- Knowledge of local services and resources available in the Birmingham area is advantageous.
- Ability to work independently and as part of a team, managing your own caseload.
