About the Role
Our client is seeking a proactive and experienced Community Support Team Leader to manage and inspire a dedicated team providing essential care and support services in Liverpool . This role involves direct supervision of support workers, ensuring the delivery of high-quality, person-centered care to individuals in their homes and community settings. You will be responsible for operational coordination, case management oversight, and ensuring adherence to all care standards and policies. Key Responsibilities
- Lead, manage, and motivate a team of community support workers, providing guidance, training, and ongoing supervision.
- Oversee the effective delivery of care packages, ensuring that individual support plans are implemented accurately and to a high standard.
- Conduct client assessments and reviews, developing and updating personalised care plans in collaboration with clients and their families.
- Ensure compliance with all relevant regulations, policies, and procedures, including safeguarding and health and safety standards.
- Manage team rotas, resource allocation, and operational aspects of the service to ensure seamless delivery of care in Liverpool .
- Act as a primary point of contact for clients, families, and external agencies, addressing concerns and resolving issues promptly and professionally.
Requirements
- Proven experience in a supervisory or leadership role within the community care or social support sector.
- Demonstrable understanding of person-centered care principles and best practices in care delivery.
- Excellent communication, interpersonal, and problem-solving skills.
- Strong organisational and time-management abilities, with the capacity to manage a team and caseload effectively.
- A recognised qualification in health and social care (e.g., NVQ Level 3 or equivalent) is highly desirable.
