Our client is looking for a proactive and organised Disability Support Coordinator to join their Community & Social Care team. This role, based in Plymouth , focuses on empowering individuals with disabilities to live independently and achieve their personal goals. You will be instrumental in assessing needs, coordinating tailored support plans, and connecting clients with the right services and resources within the community. Key Responsibilities
- Conduct comprehensive assessments of individuals' support needs and aspirations.
- Develop and implement person-centred support plans in collaboration with clients and their families.
- Liaise with a wide range of service providers, including healthcare professionals, local authorities, and community groups.
- Monitor the effectiveness of support plans and make adjustments as necessary.
- Provide information, advice, and advocacy for individuals with disabilities.
- Maintain accurate client records and contribute to team reporting.
Requirements
- Previous experience in a disability support or social care role is essential.
- Strong understanding of disability rights, inclusion principles, and relevant legislation.
- Excellent communication, negotiation, and problem-solving skills.
- Ability to build positive relationships with clients, families, and service providers.
- Proficiency in using IT systems for case management and reporting.
