Discharge Hub Coordinator – Patient Flow Specialist

Milton Keynes University Hospital NHS Foundation Trust is seeking a Discharge Officer for the Integrated Discharge Hub on a fixed-term contract or secondment. You will coordinate discharge plans on a ward, manage delays, and communicate with patients, families, and wards to ensure safe, timely discharges.

The role is Band 4 (£28,392 - £31,157 per annum, pro rata if part time) with 37.5 hours a week (or 30 hours pro rata).
