About the Role
Our client is recruiting a dedicated Mental Health Support Practitioner to provide essential support to individuals experiencing mental health challenges in the Leeds area. This hybrid role involves a combination of direct client support, potentially including home visits and community-based interventions, alongside remote case management and team collaboration. You will work as part of a multidisciplinary team, offering practical and emotional support, promoting recovery, and empowering individuals to manage their mental well-being effectively. This position is vital in ensuring accessible and high-quality mental health services within the community.
Key Responsibilities
- Provide one-to-one and group support to individuals with various mental health conditions.
- Assist clients in developing and implementing personalized recovery and support plans.
- Conduct risk assessments and contribute to safety planning for clients.
- Facilitate access to community resources, healthcare services, and social activities.
- Maintain accurate and confidential client records, adhering to professional standards.
- Collaborate closely with other mental health professionals, social workers, and support agencies.
Requirements
- Experience working within a mental health setting or with individuals facing mental health challenges.
- Understanding of common mental health conditions, recovery principles, and therapeutic approaches.
- Excellent communication, empathy, and interpersonal skills.
- Ability to balance direct client work with remote administrative tasks and communication.
- Relevant qualifications or certifications in mental health support or a related field are advantageous.
- Must be able to travel within the Leeds area as required by the role.
