Key Responsibilities

• Provide one-to-one support to clients in their homes or community to aid recovery and independence.
• Conduct risk assessments and develop support plans in collaboration with clients and mental health professionals.
• Facilitate access to community resources, services, and social activities.
• Offer emotional support and crisis intervention when required.
• Maintain accurate and confidential client records using electronic systems.
• Collaborate with a multidisciplinary team to ensure a holistic approach to care.

Requirements

• Experience working with individuals with mental health conditions is essential.
• Strong understanding of mental health recovery principles and safeguarding procedures.
• Excellent communication, empathy, and active listening skills.
• Ability to manage challenging behaviours and de-escalate situations effectively.
• Proficiency in using digital communication tools for remote support.
• Relevant qualifications such as a Level 3 Diploma in Health and Social Care or equivalent are preferred.
