Occupational Therapist - Community

About the Role

Our client is seeking a skilled and motivated Occupational Therapist to join their community-based rehabilitation team in Bristol . This role focuses on helping individuals of all ages regain or maintain their independence and quality of life by addressing physical, cognitive, and emotional challenges. You will work across various settings, including clients' homes and community centres, providing tailored interventions and support to help them achieve their personal goals and participate fully in daily life.

Key Responsibilities

• Conduct comprehensive occupational therapy assessments of clients' functional abilities, environmental needs, and goals.
• Develop and implement personalised intervention plans, utilising therapeutic activities and adaptive equipment.
• Provide education and training to clients, families, and caregivers on strategies to manage conditions and improve participation in daily activities.
• Collaborate with multidisciplinary teams, including physiotherapists, social workers, and GPs, to ensure integrated patient care.
• Evaluate the effectiveness of interventions and modify treatment plans as necessary, documenting progress and outcomes.
• Adapt environments and recommend assistive technology to promote independence and safety in the home and community.

Requirements

• Degree in Occupational Therapy and current HCPC registration .
• Proven experience as an Occupational Therapist, with a strong understanding of rehabilitation principles.
• Excellent assessment, clinical reasoning, and treatment planning skills.
• Proficiency in working with diverse client groups, including older adults, individuals with physical disabilities, and chronic health conditions.
• Strong communication and interpersonal skills to build rapport with clients and effectively collaborate with professionals.
