Outreach Support Worker - Homelessness Services

About the Role

Our client is seeking a proactive and resilient Outreach Support Worker to join their dedicated homelessness services team in Sunderland . This crucial role involves engaging with individuals experiencing homelessness, providing immediate support, and assisting them in accessing essential services and pathways to stable accommodation. You will be instrumental in building trust, offering practical assistance, and advocating for the needs of a vulnerable population. Key Responsibilities

• Conduct street outreach to identify and engage with individuals who are homeless or at risk of homelessness.
• Provide immediate support, including signposting to relevant services such as emergency shelter, healthcare, and benefits advice.
• Build rapport and establish trusting relationships with clients, understanding their individual needs and challenges.
• Assist clients in developing personalised action plans to address their housing, health, and well-being goals.
• Collaborate with partner organisations, local authorities, and charities to facilitate access to support and accommodation.
• Maintain accurate and confidential case notes, documenting outreach activities and client progress.

Requirements

• Previous experience working with vulnerable individuals, particularly those experiencing homelessness, is essential.
• Strong understanding of the issues surrounding homelessness and associated support needs.
• Excellent communication, negotiation, and interpersonal skills, with the ability to work with diverse client groups.
• Ability to remain calm and effective in challenging or high-pressure situations.
