About the Role

Our client is actively seeking a dynamic and resilient Outreach Support Worker to join their fully remote team, dedicated to serving individuals in need across the Leeds area. This role is critical for connecting with vulnerable populations who may face barriers to accessing traditional support services. Operating entirely remotely, you will utilize a variety of digital and communication channels to engage with clients, assess their needs, and link them with essential resources. Key Responsibilities

• Conduct virtual needs assessments and risk assessments for individuals experiencing homelessness, addiction, or other challenges.
• Develop and implement tailored outreach plans to engage hard-to-reach populations.
• Facilitate access to essential services such as housing, healthcare, employment support, and benefits.
• Maintain accurate and confidential client records, documenting interventions and progress.
• Collaborate effectively with partner organizations, charities, and statutory services to ensure seamless support pathways.
• Provide ongoing emotional support and advocacy for clients navigating complex systems.

Requirements

• Demonstrable experience in an outreach , support work , or social care role.
• Strong knowledge of issues affecting vulnerable populations in the Leeds region.
• Excellent communication and interpersonal skills, with a proven ability to build trust remotely.
• Proficiency in using digital tools for communication, case management, and data entry.
• Ability to work independently, manage time effectively, and maintain professional boundaries.
• A proactive approach to problem-solving and resourcefulness in connecting clients with services.
