About the Role
Our client is seeking a dedicated Outreach Support Worker for their hybrid role in Bristol, South West England, UK . This position involves proactively engaging with individuals within the community who may require support but are not actively seeking services. You will play a critical role in building trust, assessing needs, and connecting individuals with the appropriate help, utilizing both field-based and remote communication methods. This is an opportunity to make a direct impact on individuals facing various challenges, including homelessness, substance misuse, or social isolation.
Key Responsibilities
- Conduct proactive outreach in the community to identify and engage with individuals in need of support.
- Build rapport and trust with clients, understanding their unique circumstances and needs.
- Conduct initial assessments and develop basic support plans for individuals.
- Connect clients with relevant services, such as housing, health, employment, and social support.
- Provide ongoing follow-up and support through phone calls, virtual meetings, and in-person visits.
- Maintain accurate records of outreach activities, client interactions, and service referrals.
Requirements
- Previous experience in outreach, community work, or a similar support role.
- Excellent interpersonal and communication skills, with the ability to engage diverse populations.
- Understanding of issues such as homelessness, addiction, and social exclusion.
- Ability to work independently, plan outreach routes, and manage time effectively.
- Proficiency with IT systems for case recording and communication.
- A driving license and access to a vehicle for community travel are often required.
