This role focuses on engaging with individuals who may be experiencing homelessness, substance misuse issues, or other complex social challenges, providing essential support and guidance to help them access services and move towards stability. You will be instrumental in building trust, offering practical assistance, and connecting clients with appropriate pathways to recovery and support networks within the Hampshire community. Key Responsibilities
- Proactively engage with individuals in the community who are facing complex social challenges.
- Provide practical support such as signposting to housing, health services, and employment opportunities.
- Build trusting relationships with clients, offering emotional support and encouragement.
- Assist clients in developing personal action plans to achieve their goals for stability and independence.
- Maintain accurate and confidential records of interactions, interventions, and client progress.
- Collaborate with partner agencies and support services to ensure a coordinated approach to client care.
Requirements
- Previous experience in outreach, homelessness services, substance misuse support, or a related field.
- Excellent communication, listening, and interpersonal skills.
- A non-judgmental attitude and a commitment to promoting client autonomy and dignity.
- Ability to work independently and as part of a team, often in challenging environments.
- Knowledge of local support services and referral pathways is an advantage.
