About the Role

Key Responsibilities

• Proactively engage with individuals in the community who are facing complex social challenges.
• Provide practical support such as signposting to housing, health services, and employment opportunities.
• Build trusting relationships with clients, offering emotional support and encouragement.
• Assist clients in developing personal action plans to achieve their goals for stability and independence.
• Maintain accurate and confidential records of interactions, interventions, and client progress.
• Collaborate with partner agencies and support services to ensure a coordinated approach to client care.

Requirements

• Previous experience in outreach, homelessness services, substance misuse support, or a related field.
• Excellent communication, listening, and interpersonal skills.
• A non-judgmental attitude and a commitment to promoting client autonomy and dignity.
• Ability to work independently and as part of a team, often in challenging environments.
• Knowledge of local support services and referral pathways is an advantage.
