About the Role

Our client is seeking a highly organized and compassionate Remote Care Coordinator to join their fully remote team, supporting individuals across the UK. In this pivotal role, you will be instrumental in ensuring clients receive the appropriate support services tailored to their unique needs, all managed from the comfort of your home office. You will act as a key point of contact, facilitating communication between clients, their families, and a network of external care providers. Key Responsibilities

• Coordinate and arrange a range of support services for clients, including domiciliary care, therapeutic interventions, and social activities.
• Conduct virtual needs assessments and develop comprehensive, person-centred care plans.
• Liaise effectively with clients, their families, and a diverse network of external service providers via phone, email, and video conferencing.
• Monitor the delivery and effectiveness of care plans, making necessary adjustments to ensure client satisfaction and well-being.

Requirements

• Previous experience in care coordination, case management, or a similar role within the social care sector.
• Strong IT proficiency, including experience with virtual communication tools and CRM systems.
• Excellent organisational and time-management skills, with the ability to manage multiple cases remotely.
• Exceptional communication and interpersonal skills, with a empathetic and professional approach.
• Ability to work autonomously and proactively in a remote setting.
