About the Role
Our client is seeking a highly organised and motivated Remote Care Coordinator to join their expanding virtual support team. This role is fully remote, offering the flexibility to work from anywhere within the UK. You will be responsible for managing and coordinating care plans for clients remotely, ensuring they receive timely and appropriate support services. The focus is on leveraging technology to connect clients with resources, monitor their well-being, and facilitate communication between clients, families, and healthcare providers, all from the comfort of your home office.
Key Responsibilities
- Coordinating and managing client care plans through virtual communication channels.
- Liaising with clients, families, and external service providers to ensure seamless care delivery.
- Monitoring client progress and well-being through regular remote check-ins and digital platforms.
- Facilitating communication and information sharing between all involved parties.
- Maintaining accurate and confidential electronic client records.
- Identifying potential issues or barriers to care and proactively seeking solutions.
Requirements
- Previous experience in care coordination, case management, or a similar support role.
- Strong understanding of care provision and community support services.
- Excellent digital literacy and comfort with virtual communication tools (video conferencing, messaging apps).
- Exceptional organisational and time-management skills.
- Ability to work autonomously and maintain high levels of professionalism in a remote setting.
