About the Role
Our client is looking for an organised and empathetic Remote Care Coordinator to join their virtual team, supporting individuals across the UK. Key Responsibilities
- Coordinate and schedule a variety of support services, ensuring timely and efficient delivery based on individual care needs assessments.
- Act as the main point of contact for service users, their families, and external agencies, addressing queries and resolving issues promptly and professionally.
- Maintain accurate and up-to-date electronic records of all communications, service provision, and client progress.
- Liaise effectively with care staff to provide clear instructions, gather feedback, and address any challenges they encounter.
- Assist in the development and regular review of personalised care plans, ensuring they remain relevant and effective.
- Monitor service delivery KPIs and report on performance metrics to management, identifying areas for improvement.
Requirements
- Proven experience in a coordination, administrative, or customer service role, preferably within the care sector.
- Excellent organisational and time-management skills, with the ability to multitask effectively in a remote setting.
- Strong IT proficiency, including experience with scheduling software, CRM systems, and Microsoft Office Suite.
- Exceptional written and verbal communication skills, with a patient and empathetic approach.
- Ability to work independently, demonstrate initiative, and maintain a high level of professionalism remotely.
