Our client is seeking a proactive and organised Remote Care Coordinator to manage and enhance the support services delivered to clients, entirely from a remote setting. Key Responsibilities

• Develop, monitor, and coordinate individualised care and support plans for clients from a remote location.
• Act as a key liaison between clients, their families, external agencies, and internal support teams.
• Utilise digital platforms and communication tools to manage client needs and service delivery effectively.
• Conduct regular remote check-ins and assessments to ensure client well-being and satisfaction.
• Troubleshoot issues related to care provision and facilitate solutions in a timely manner.
• Maintain accurate and up-to-date electronic records of all client interactions and service arrangements.

Requirements

• Proven experience in care coordination, case management, or a similar role within health or social care.
• Strong organisational and time-management skills, with the ability to manage multiple priorities remotely.
• Excellent communication, interpersonal, and digital literacy skills.
• Familiarity with care management software and virtual communication tools.
• Ability to work autonomously and demonstrate initiative in a remote work environment.
• A background in social care, nursing, or a related field is advantageous.
