Key Responsibilities

• Liaise with local community organisations and service providers to facilitate client referrals and ensure coordinated support.
• Maintain accurate client records and update case notes diligently using the organisation's digital platform.
• Offer empathetic listening and practical guidance, empowering clients to access the support they need independently.

Requirements

• Experience in a support role , advisory capacity, or customer service role, preferably within the community or social care sector.
• Excellent communication and active listening skills, with the ability to build rapport remotely.
• Strong knowledge of local community resources, social services, and welfare rights.
• Proficiency in using digital communication tools and case management software.
• Ability to work autonomously, manage time effectively, and maintain productivity in a remote setting.
• A patient, empathetic, and proactive approach to supporting diverse client needs.
