About the Role

Key Responsibilities

• Coordinate and manage support plans for a caseload of clients entirely through remote channels.
• Conduct virtual needs assessments and regular check-ins to monitor client progress and well-being.
• Liaise with a network of external agencies and service providers via email, phone, and video calls.
• Provide clients with information, advice, and signposting to relevant community and social services.
• Maintain accurate and up-to-date digital records of client interactions and support activities.

Requirements

• Experience in social care, community support, or a related field.
• Demonstrated ability to effectively manage client cases and coordinate support remotely.
• Excellent digital communication skills, including proficiency with virtual meeting platforms and case management software.
