Senior Care Lead: Dementia & Personalised Care

HC-One is seeking a Senior Care Assistant to lead a team in dementia, nursing, residential and specialist care homes. You will plan and implement person-centered care, build strong relationships with residents and families, and work closely with the Registered Nurse and Care Manager to ensure high standards of care.

You will support residents with daily activities, maintain accurate records, and help deliver a range of social and recreational activities to promote dignity and independence.
