Senior Transitions Manager – Young Adults Services

Lancashire County Council is seeking an experienced Transitions Manager to lead a Young Adults Service team within Adult Social Care. You will oversee 18–25 year olds’ transitions, promote independence, and ensure high-quality, person-centred services.

The role requires strong leadership of Social Workers and staff, safeguarding, assessments, and joint working with education, health and services to help young adults maximise their independence and achieve their aspirations.
